Please know that our hospice patients and families lead the care provided by our team and we work collaboratively to ensure comfort and symptom management. Our hospice team consists of a Physician, a Registered Nurse Case Manager, a Hospice Aide, a Social Worker, and Chaplain. These individuals are here to assist you and your family during this fragile time, and we are ready to answer any questions you may have.

We are proud to have some of the best ratios for patient-to-staff in the industry. What does that mean for the patients and families of our hospice care? It results in more hospice visits and better bedside care to offer the best support and comfort. We strive to always keep our hospice patients at home and to avoid any visits to the ER or rehospitalizations. Our patient visits remain focused and frequent for more quality time at the bedside and better communication with every patient and their family.

Our Houston staff is based in the community and are dedicated to providing amazing hospice care to the great Texans of this area. Our ownership is local, and that means we can make quick decision, have great resources, and react to any immediate concerns, including hospice care situations that can arise.

Bayou City Hospice has developed great relationships with the local facilities that offer healthcare. Our patients that are in Assisted Living or Long-Term Care facilities will experience excellent care services because of our great partnerships. We strive for effective coordination and also communication with staff and Houston hospice employees. We make the time to talk with our partners at these facilities to make sure we meet the needs of our patients and families.
Being There When It Counts
When the hospice patients we have begin to decline and require more support, we maximize the patient visits and can even provide around-the-clock hospice care support when it is deemed necessary. Our low caseloads allow our Houston staff to be fully present and provide the best support to not only the hospice patients, but their families as well.
24/7 Support Team
Bayou City Hospice has a special call team for after hours because we realize that there can be situations that arise that require around-the-clock support. We have a call line with 24 hour support by a Nurse Manager and Houston field staff team to provide support for patients or family at any point during the day or night. Our patients and families can be comforted knowing we are available to them at any time. When you receive hospice care from us, you get full support.
