Our mission is to exceed the expectations of those we serve, and we will always do our best to find ways to say yes. Please know that our patients and their families lead the care provided by our team and we work collaboratively to ensure comfort and symptom management. Our hospice team consists of a Physician, a Registered Nurse Case Manager, a Hospice Aide, a Social Worker, and Chaplain. These individuals are here to assist you and your family during this fragile time, and we are ready to answer any questions you may have.

The Team That Cares

Our care is provided by a team of trained and experienced professionals – physicians, nurses, counselors, social workers, aides, and volunteers – who provide medical care and support services not only to the individual, but to the patients’ family and loved ones.

A Dedicated Hospice Care Team

Bayou City Hospice has some of the lowest patient to staff ratios in the industry. How does that impact our patients and families? It means more hospice visits and more care at the bedside to provide the comfort and support they deserve. We work hard to keep our patients at home avoiding ER visits and rehospitalizations. Frequent and focused patient visits mean more quality time at the bedside and better communication with our patients and families.

Our Houston staff has deep roots in the community and are dedicated to delivering the best evidence-based hospice care to our fellow Texans. With local ownership, we can make decisions quickly, have great resources, and are able to respond to immediate concerns, including hospice care situations that can arise.

Collaboration in the Houston Community

Bayou City Hospice has developed great relationships with other healthcare facilities and the medical community. Our patients in Assisted Living or Long- Term Care communities receive excellent end of life care services through our partnership, coordination and collaboration with facility staff and Houston hospice employees. We have the time to communicate with our facility partners to make sure we meet the needs of our patients and families.

Being There When it Matters

When our hospice patients begin to decline and require more support, we increase our patient visits and can even provide 24/7 continuous hospice care support if deemed medically necessary. Our low caseloads allow our Houston staff to increase their presence to support our hospice patients and families in crisis.

After Hours Support Team

Bayou City Hospice has a special after hours on call team because we realize that situations arise for our patients and families that require support 24/7. We have a 24 hour call line supported by a Nurse Manager and Houston field staff team to provide patient and family support any time day or night. Our patients and families can be comforted knowing we are available to them at any time. When you receive hospice care from us, you get full support.
