Patient-centered care means we believe that our care is defined by the wishes and needs of patients and their families. Our commitment to our patients and families that we serve is to help the patient remain wherever they call home and avoid unnecessary hospitalizations that cause stress and hardship. Bayou City Hospice strives to provide the best Hospice care for patients and families in Houston and the surrounding communities.

Locally owned and operated, we have deep roots in the community. We believe that great care is centered in giving our experienced clinicians the time and resources necessary to provide the highest level of quality care. Home Care

Provided wherever our patients call home including private residences, assisted living, nursing homes, or residential facilities.

Visits are made to meet the medical, spiritual, and emotional needs of our patients and families.

Patient needs are met through scheduled and on-call visits up to 7 days a week

Our patient’s comfort level is our number one goal. Through our thorough and supportive staffing model, we can increase how often we visit when clinically necessary to keep our patients comfortable. Our expert staff and their visit frequencies allow us to manage symptoms and avoid any crises that may arise.

General Inpatient Care

Temporary/short term care provided in a skilled nursing facility or hospital when a patient’s symptoms cannot be medically managed at home.
Our goal is always to keep our patients wherever they call home, but we contract with facilities when the need for inpatient stay is necessary.

Appropriateness for General Inpatient Care is evaluated daily by an RN.

Our goal is to get the patient’s condition stabilized so they can return home as soon as it is clinically appropriate.

Respite Care

Respite Care

Caring for a loved one with serious health issues can be exhausting for the primary caregiver and other family as well. We understand that and take that very seriously.
The Medicare Hospice Benefit allows a hospice patient to stay for up to 5 days at a hospice inpatient unit or a nursing facility.
Respite Care gives a home patient’s family time to attend out-of-town events (graduation, wedding etc.) or take a much-needed break to rest.
Our clinical and spiritual care team can assist families in finding appropriate respite care facilities.

Continuous Care

Temporary/short-term care is provided in the patient’s home when symptoms are not well controlled through normal visits.
Nurses or Home Health aides are staffed at the bedside for up to 24 hours a day. Appropriateness for Continuous Care is evaluated daily by an RN.

Continuous care nurses communicate with our physicians and RN Case Managers to address changes and manage the patients’ condition.

Continuous Care provides the level of support patients and families need to feel comfortable in their homes during a crisis. Bedside support by our staff maintains the comfort of the patient clinically and provides emotional support for the family. This creates an environment where families can spend time with their loved one.

Bereavement Services

Bereavement Services

We recognize the difficulty with losing a loved one and so we provide up to 13 months of bereavement support.
We provide mailings every few months and occasional phone calls to check and see how you are coping.
If you need more support we can talk with you by phone, meet with you wherever is comfortable for you, and also provide referrals to other resources.
An annual Memorial service is offered once a year to give you another opportunity to honor and remember your loved one.
We are just a phone call away for tough times like birthdays, anniversaries, or any other time you need us.
