We believe that no matter where a hospice patient is staying they should feel like they are home, so we provided home health visits, hospital visits, general care at nursing homes or other long-term facilities, as well as impatient care. We strive to avoid hospitalizations that are unnecessary and can be stressful and draining and do our best to continue the best level of care possible without defaulting to a hospitalization. Our hospice patients are our number one priority whether they have entered into our hospice care, palliative care, require regular home health checkups, or are in a long-term care facility. Regardless of where our hospice patients call home we provide the same quality of hospice care. Whether they’re in a residential facility, nursing home, assisted living, hospital inpatient facility, or private family residence we strive to make every patient feel at home through the support and care of our staff. These routine hospice healthcare visits are designed to not only meet the immediate medical needs of our patients, but also their spiritual and emotional needs. The family members of our patients are an extension of our care and support and can benefit greatly through these regular visits. These visits will be scheduled to provide regular care and these patient services may be scheduled up to 7 days a week. 

Navigation Comfort

With hospice care and many of our additional support services our main goal is the comfort of our patients. To best navigate our options for patient comfort we are able to increase the frequency visits our support staff makes in a week. When symptoms increase we are able to make more visits and can provide more extensive support. This flexibility means less worrying for the family and better medical resources during times when symptoms feel overwhelming.

When dealing with medical uncertainty Bayou City Hospice knows that the required support a hospice patient needs can change frequently. We strive to make hospice care in Houston as accommodating as possible for the family members and home care providers of each patient. Our staff is ready to provide short-term care in a nursing facility, or even a hospital during times when a hospice client is showing symptoms that aren’t as easily managed at home. Wherever our hospice patients are we work with healthcare providers and facility staff to make their stay as comfortable as possible. It is a crucial part of the Bayou City Hospice belief that wherever a patient is located they should feel they are home. In order to make sure the care a patient is receiving is at the appropriate level, an RN will perform regular evaluations to help our staff determine what is best. We always consult with the patient and their loved ones before making any changes to their hospice care they are receiving. Short-Term Respite Care

In many hospice situations health complications can be overwhelming and truthfully can become exhausting for the family and other primary caregivers. Oftentimes caregiving requires more time and energy than an individual has to offer and they will require both support and a way of taking much needed personal rest. Respite Care gives the patient’s family a short time to step away from daily care in order to attend an event such as a wedding, graduation, or an out of town obligation. If the family simply needs time to rest and recharge, they are able to take advantage of our respite care support service knowing their loved one is receiving excellent quality care. As a part of the Medicare Hospice benefits a patient is able to stay comfortably in an inpatient facility for hospice patients or a nursing home for up to 5 full days. A temporary caregiver providing full-time support for your loved one provides necessary time for family members to take care of themselves so they have the strength to care for their loved one. Continuous Care

During periods where there is more difficulty controlling symptoms we offer continuous care, which serves as a temporary, short-term service that relieves the pressure of caretakers and family members during a potential crisis. A healthcare worker such as a nurse or an aid will provide bedside support for your loved one. This care can be up to 24 hours per day and one of the best benefits of this is that it allows the family members of the patient more quality time to spend with their loved one instead of putting their energy towards caregiving. An RN will monitor and evaluate when continuous care is appropriate daily and provide feedback to our staff. 

Palliative Care

There are many types of home health care, including Palliative care, a form of long-term care that aims to support families and their loved ones who are living during times of serious illness. During palliative care, our healthcare professionals will focus on overall quality of life by improving general comfort, and aiming to increase the ease of daily life for the patient. Beyond easing physical discomfort, during palliative care nurses also may assist with the daily practical needs of a patient. Palliative care is often combined with other forms of primary medical care; this type of care is not strictly end-of-life care and can be largely beneficial for patients in different stages of their illness. Bereavement Services

Bereavement Service

Losing a family member or loved one is a very difficult process and in order to help provide as much support as possible we offer up to 13 months of support for bereavement. Bayou City Hospice provides a variety of resources and will continue making contact with you during your time of bereavement. We send mailings multiple times throughout the year, offer phone support and can provide further support if you would like both on the phone and in-person. We can meet you in a place of your choosing that will help you feel comfortable and are able to provide additional resources and referrals if you feel you can benefit from them.
